I spent three days chasing a bug where a customer support agent kept referencing a pricing policy from six months ago. The logs showed the correct policy was in the context window. The retrieval system had surfaced it. The model just kept citing an older version stored in episodic memory. This is what I call a memory attribution failure.
Attribution failures happen when an agent cannot correctly connect a piece of information to its source, timestamp, or scope. The agent retrieves something real. It acts on something wrong. The output looks confident. The error only surfaces when a human reviews the citation.
What Attribution Actually Means in Agent Memory
Attribution has three components. The agent needs to know what it knows, when it learned it, and whether that knowledge applies to the current task.
Episodic memory stores sequences of interactions. When an agent retrieves from episodic memory, it pulls a compressed representation of past events. The compression drops metadata. The timestamp goes missing. The context of how the interaction ended gets simplified. Semantic memory stores structured facts about domains, policies, and entities. When the agent retrieves a fact from semantic memory, it often loses the provenance chain, which version of the policy it refers to, and whether the policy has been superseded.
Working memory holds the current session state. Attribution failures in working memory look different from episodic failures. The agent treats a transient inference as a grounded fact. It conflates a hypothesis generated in-context with information retrieved from a persistent store.
The third component trips up most teams. Most retrieval systems return the most relevant chunks. The agent then treats those chunks as authoritative. Relevance is not the same as recency, and it is not the same as applicability. A chunk about pricing from Q3 2025 might outrank a chunk about Q1 2026 pricing because the older chunk has richer surrounding context that Retrieval Augmentation systems latch onto.
The Failure Modes I Keep Seeing
The first failure mode is temporal conflation. The agent retrieves a fact from semantic memory and treats it as current. I traced this in a multi-agent pipeline where the supervisor agent maintained a policy table in semantic memory. The table was updated quarterly. Between updates, the supervisor kept routing requests using stale thresholds because no freshness metadata was attached to the retrieved values. The fix was straightforward. The detection was not.
The second failure mode is source conflation. This shows up in multi-agent setups where multiple agents write to a shared episodic store. Agent A retrieves a plan step that Agent B generated. Agent A cannot tell whether the plan step came from its own prior reasoning or from another agent. It treats the plan as grounded when it is actually a suggestion from a peer that was never ratified. In production, this manifests as agents second-guessing themselves or executing steps out of order because they cannot distinguish between self-generated and peer-generated content.
The third failure mode is scope misapplication. The agent retrieves a piece of information that applies to a different domain, product line, or customer tier. The context window has no mechanism to flag scope boundaries during retrieval. The model fills the gap with plausible inference. The plausible inference is wrong. A customer support agent that serves multiple tiers will retrieve a benefit description from one tier and apply it to a request from a different tier unless scope filtering is explicit in the retrieval query.
Why Standard RAG Does Not Fix This
RAG systems optimize for retrieval relevance. They rank documents by embedding similarity and return the top-k results. Attribution failures in RAG-driven agents come from what happens after retrieval. The model receives the retrieved chunks. It reads them. It generates an answer. Nothing in that pipeline forces the model to track which chunk supported which part of the generated response.
The rag-evaluation-metrics-what-actually-matters framework measures retrieval accuracy and answer quality. It does not measure attribution accuracy. A system can score well on RAG benchmarks and still produce confident responses built on misattributed chunks.
Most production RAG implementations I have reviewed lack attribution metadata in the vector store. The chunks have IDs, document names, and maybe section headers. They rarely have effective timestamps, scope flags, confidence scores from the embedding model, or provenance chains that track which agent wrote the chunk and when. Without that metadata, the retrieval query cannot filter by recency or scope. The model has to infer these properties from context, and it infers them imperfectly.
What I Found Works
Attaching provenance metadata to memory chunks is the first step. Every chunk in the vector store gets a payload that includes the write timestamp, the source agent ID, the target scope (which product, tier, or domain the chunk applies to), and a validity window. The validity window is the most important field and the most missing one. Most teams do not store when a piece of information stops being valid. They rely on version numbers, but version numbers require consistent versioning discipline across every system that writes to memory.
I found that episodic-vs-semantic-vs-working-memory-agents clarifies which memory system should own provenance metadata. Episodic memory entries get timestamps automatically. Semantic memory entries need explicit scope and validity fields. Working memory is session-scoped and does not need long-term provenance, but agents need a clear protocol for promoting working memory inferences to semantic memory with full attribution metadata.
The second intervention is query-time scope filtering. When the agent retrieves from memory, the retrieval query needs to include scope parameters that the agent populates from the current task context. The agent knows which product, region, and customer tier the current request belongs to. That information should appear in the retrieval query, not just in the prompt context.
The third intervention is attribution verification before response generation. After retrieval but before generating the final response, the agent runs a verification step that checks whether each retrieved chunk has a valid scope match with the current request. Chunks that fail the check get downgraded or excluded. I implemented this as a lightweight filter layer in production-ai-agent-errors patterns, applied after retrieval and before context assembly.
The Deeper Problem
Attribution failures are not a retrieval problem. They are a memory design problem. Most agent architectures treat memory as a passive store. The agent retrieves and acts. The store does not enforce scope boundaries, temporal validity, or source provenance. The model is expected to infer these properties from context, but the context window has no guaranteed structure for this inference.
Building memory systems that enforce attribution requires treating memory as an active component, not a passive store. Memory writes need schemas. Memory entries need metadata. Memory reads need filters. The agent needs to interact with memory through an interface that enforces these constraints, not through raw vector similarity search.
I kept seeing teams add more context to the prompt to fix attribution failures. More context does not fix attribution failures. More structure in memory does.
