Across two years of debugging production AI agents, I keep landing on the same handful of failures. Different companies, different frameworks, same root causes. The field is starting to converge on solutions, and most teams are still catching up.
These failures cluster into four categories. I wrote about the specific error patterns in my post on production AI agent errors if you want the taxonomy. The piece you are reading now is about why the failures happen in the first place and what actually helps.
Agents fail because the loop is not where you think it is
Most engineers assume the agent loop runs in their code. It does not. The loop runs inside the model's forward pass. Your code orchestrates around it, and the actual decisions happen inside a black box you cannot step through with a debugger.
Two consequences follow from that. The model can drift mid-loop. I once watched a code-refactor agent start out renaming a function, get three files deep, then quietly reframe its own job as "clean up the imports" because a long file of tool output had pushed the original instruction out of its working attention. The second consequence is that you cannot set a hard budget on loop iterations the way a while counter caps a normal loop. The model decides when it is done, and it can be wrong about that.
Reflex agents and deliberative ones fail differently, a split the taxonomy of AI agents I wrote covers in structural detail. A reflex agent that misfires usually does one wrong thing and stops. A deliberative one builds a wrong plan and then executes ten coherent steps toward the wrong outcome. You need to know which category you are building before you can predict how it will break.
Context window exhaustion is the silent killer
Agents accumulate state in the context window. The longer a session runs, the more tokens get spent on history, tool results, and intermediate reasoning, until the window fills up. Think of it like a whiteboard the agent never erases: every tool call and every reply gets jammed onto the same surface, and once the board is full, older notes get wiped to make room whether or not they still matter.
A customer support agent I watched handled the first 40 turns of a conversation cleanly. Around there the context began compressing, the model lost access to the opening of the chat, and the agent started giving answers that contradicted what it had promised the customer 30 turns earlier. It was not lying. It literally could no longer see its own earlier replies.
Short-term memory for AI agents covers the mechanics of how this happens. Bigger context windows look like the obvious answer and they are the wrong one, because doubling the window roughly doubles the per-call inference bill on every turn, which gets brutal once you run thousands of concurrent sessions. What works is explicit memory management with eviction policies, the kind of thing most frameworks implement badly or skip entirely.
Multi-agent systems amplify every failure mode
Splitting a task across multiple agents does not hand you one agent's reliability. You get the product of all their reliabilities, the same way a chain of five hand-offs is only as sound as every link surviving. Five agents each running at 90% reliability multiply out to 59% overall. That is not a production system.
Teams scale to multi-agent architectures because they want parallelism or role specialization, and then they forget to budget for the coordination overhead. When the tradeoffs actually pencil out is exactly what Multi-agent versus single-agent tradeoffs works through.
Three failure modes dominate the multi-agent systems I see. Context pollution, where one agent's scratch work leaks into another's prompt and muddies its reasoning. Cascading errors, where a planner agent hands a researcher agent a subtly wrong task and the researcher dutifully executes it. And silent agreement on wrong answers, where two agents talk past each other and settle on a confident, incorrect conclusion that neither would have reached alone.
Tool use introduces failures your tests never catch
Agents call tools, tools fail, and the failure modes are mundane right up until they hit production. A timeout is the classic one. The tool call hangs, the agent retries, the retry succeeds, and because the agent never checks whether the first call already landed, it double-executes. A refund gets issued twice. A Slack message goes out twice. A database row gets incremented twice. None of this shows up in a happy-path test, because in tests the tool never times out.
Prompt injection sits in a nastier class of tool failure. An adversarial input riding in through the agent's context can hijack a tool call, the way a forged note slipped into an assistant's inbox could get them to wire money if they trusted every instruction they read. I have watched this happen for real on an agent that summarized inbound support tickets, where one ticket body contained instructions aimed squarely at the model rather than the human.
The Model Context Protocol was designed partly to fence off this class of failure, though protocol compliance still varies a lot across implementations.
What the field is doing about it
Solutions are emerging, just unevenly distributed across the teams that need them.
Circuit breakers for agents are starting to show up. Borrowed straight from the resilience pattern that trips a service offline when downstream calls keep failing, agent circuit breakers halt the loop once the error rate crosses a threshold, so a stuck retry loop cannot keep firing actions for an hour. Few production teams have these yet, and the pattern is simple enough to write yourself in an afternoon.
Observability keeps improving. The agent observability stack I reviewed covers the tools that surface what the agent is actually doing inside the loop, every prompt, every tool result, every retry. Run an agent without that and you are debugging a crash with no stack trace.
Context management is finally getting serious attention. Teams are walking away from naive accumulation toward structured memory hierarchies with explicit eviction, deciding on purpose what stays in the window and what gets summarized or dropped. What you should do today

Add circuit breakers before you scale the agent count. Add observability before you add the next agent. Those two investments have headed off more production incidents, in my experience, than any amount of prompt tuning.
Agents are not failing because the technology is immature. They fail because the operational patterns around them are still being invented. What I see in production, again and again, is teams applying LLM-era intuition to what is, underneath, a distributed systems problem: timeouts, retries, partial failure, state that drifts out from under you. The fixes already exist. Knowing which ones apply to your situation is the work that remains.
